Money Transfer Booklet

I am trying to find out more about this book that belonged to my second grandfather. He came from Lithuania and received this booklet. It says on the cover "Money Transfer Booklet". I’ve tried searching online and can’t seem to find a picture of this booklet or any information about it. I’m just wondering if this is the only booklet left? It now goes by the name and address of 1739 S Halsted St Chicago Illinois. This address used to house a publishing house of Lithuanian newspapers "News" and I think this booklet was from them. Also, you see a flower in the middle of the booklet, the meaning of which I can’t find? Does anyone know what it could be?
